1. Risks. Dancing is physical activity. It carries risks, including slips, falls, collisions with other dancers, over-exertion, strains, sprains and fractures, and in rare cases serious injury or death. I am taking part voluntarily and I accept these risks.
2. Release. To the fullest extent Idaho law allows, I release Dynasty Ballroom LLC, its owners, instructors, employees, volunteers, and the operators of any venue it uses (together, “the Studio”) from all claims for injury, illness, or loss of or damage to property arising from my lessons, classes, practices, or events, including claims based on the Studio’s ordinary negligence. This does not release the Studio from gross negligence or willful misconduct.
3. My responsibility. I will follow my instructor’s guidance and dance within my limits. I agree to cover the Studio’s costs, including reasonable attorney’s fees, for any claim by someone else that is caused by my own actions.
4. Health. I am physically able to dance. I will tell my instructor about any condition, injury, or pregnancy that could affect me, and I will stop if I feel unwell.
5. Emergency care. If I am hurt and cannot decide for myself, I authorize the Studio to call for and obtain emergency medical care for me, at my own expense.
6. Minors. If the participant is under 18, the parent or legal guardian signs below, agrees to this waiver on the participant’s behalf, and confirms the participant is able to take part.
7. General. This waiver covers all of my participation with the Studio until I withdraw it in writing. It is governed by Idaho law, and any dispute will be heard in Bonneville County, Idaho. If any part is found unenforceable, the rest still applies. I have read this waiver, I understand it, and I am signing it voluntarily.
